Forms & Docs

Grab your favorite pen and a steaming café au lait and let's get started!

Here is the link to the Checklist for Application for the list of additional documents required.  After you've completed those tasks, check out the Checklist for After Acceptance list for more info.  The Forms & Docs tab on this website will become your new bestie.

Application Items
These items will need to be emailed to Montpellier@SCLou.org.  Send each document in a separate email.  Please use the following format for the subject line of your email. (You or your professors can hand-deliver the Letters of Recommendation if you prefer)

MLIE - document name - your name

  • Application in French - Link to application in French to be completed in French
  • Resume in English - This is your regular resume.  If you don't have one, the Career Center at your school can assist you in creating one.
  • Cover Letter in English - This is a cover letter specific to the program.  If you don't have one, the Career Center at your school can assist you in creating one.
  • Lettre de Motivation - This is a cover letter in French, again, specific to the program.
  • Unofficial Transcript - For students at UofL, you can download this instantly by logging into your ULink account, click the Student Services tab, scroll down to Undergraduate (or Graduate) Advising, and clicking on View my Unofficial Transcript.  Save as a pdf and email.
  • Letters of Recommendation - You will need a recommendation from TWO different professors or faculty at your university.  See the Checklist for After Acceptance for more info on this.
  • Graduate School Acceptance Letter - This is for grad school students only.

After Acceptance Items
Please carefully read each item on this list.  You will have some that take more time to complete than others.  Plan ahead!  Some of these items will be filled out at the mandatory Orientation Meeting.
  • Banking Info - You will need to provide direct deposit information in order to get paid for working in Montpellier.  You'll need to contact your bank directly, and possibly go to a branch, in order to get the necessary information.  There are two ways to get this information from your bank.
    • Some banks readily provide this information or will type something up on letterhead at your request.  If so, this is a template for them to follow:  Banking Info on Letterhead
    • Other banks will not perform this task for you.  There is a workaround by asking them to print their ABA and Swift Code information on letterhead (they will oftentimes have a generic form available) as well as a deposit account balance summary with your routing and account number on it.  You can block out the personal account balance information.  Here is an example of a General Swift Code Info and Account Balance Summary HERE that another student used.
    • Here's what information you must have:
      • Name on the account
      • Routing Number
      • Account Number
      • Swift Code (for international transfers)
      • Completed on bank letterhead
  • MLIE Information Form - Filled out at the mandatory Orientation Meeting
  • Release and Assumption of Risk - Filled out at the mandatory Orientation Meeting or you can print the Release and Assumption of Risk form and take with you to Orientation.
  • Student Conduct Agreement - Filled out at the mandatory Orientation Meeting or print the Student Conduct Agreement and take with you to Orientation.
  • Health Clearance - This is a two-page form to have your doctor fill out for you, stating that you are fit for travel.  There are a couple of sections for you to sign as well.  Plan ahead with enough time to make an appointment with your doctor!  Submit via email.
  • Passport Photos - You will need two passport photos.  You can get these taken at CVS, Walgreens, Wal-mart, or even the Post Office.  These are not for your passport.  Bring the photos with you to the mandatory Orientation Meeting.
  • Color Scan of Passport or Passport Application - This another thing you'll need to plan well in advance for if you do not already have a current passport.  Visit the link to Passports for more information on getting one.  An example of an acceptable Passport Scan is here.  Submit via email.
  • Travel Information Form - Once you have your travel plans secured (flights and trains), please fill out the Travel Information Form.  Submit via email.
  • Travel Advisory Form - The Travel Advisory Form will be provided at Orientation or you may print, fill out, and bring to Orientation with you.
  • Demographics Form - The Demographics Form can be found here.
  • Emergency Contact Form - The Emergency Contact Form can be found here.
  • PEAP (Personal Emergency Action Plan) Form - The PEAP Form can be found here.
